I bought mine from Sherline or Smithy a few years back. I think www.lindsaybks.com (http://www.lindsaybks.com) might have it.

Try TEE Publishing in England. They take plastic, and it's easy to charge stuff. The credit card company takes care of the pounds/dollars conversion at an excellent exchange rate. I'm not sure they have it, but I wouldn't be surprised.

Or just try some of the HSM and Live Steam advertisers. I think Power Model Supply used to carry it; are they still around?

Try 'abebooks.com'. They search for all out-of-print books.
