I need some threaded studs that are different diameters on each end. 5/16" on one end 3/8" on the other.
Anyone ever see such a thing? and if so what do you call them?
If I have to I will make them.

Try McMaster - or a real auto parts store. I remember seeing the stepped studs in the Dorman drawers of automotive fasteners many years ago.

Well I searched Dorman, Mcmaster, Grainger, and Fastenall.
No luck. What do you call the d**m things?
I know that someone makes them.

i have seen them in old small engines i scrapped, they were i think head bolts/studs.

We used to call them 'bull studs'. I think I have seen them called 'double diameter studs". They used to be pretty common to repair stripped holes for engine manifolds.
