I have two of these, NOS/NIB. Here is what appears to be the exact same item on ebay, sold from the Ukraine:

In one of the pictures you can see the manufacturer's logo imprinted on the part to the left of the model number.

Anyone recognize this?

Probably Sumitomo.

Pretty old - separate "chip breaker" etc. Might be good for brass (0 degrees rake etc). I have a few similar.

Probably Sumitomo.

The logo looks quite different to Sumitomo , and the Sumitomo gear I have is well finished with no rough machining marks visible.

The tool is in Russia ...so it could well be a Russian manufacturer.

Yeah, now that I look again I agree it's not Sumitomo because their logo is more like a snowflake design.

I have a Valenite toolholder of the same type in an ANSI designation, a CTGPR 163D and it's nicely finished. The TPG inserts I use in it are a Valenite VC610 cermet, used for finishing at very high speeds and leaves quite a nice finish. Poor chip control with a mechanical chipbeaker, so not good for materials that make long stringy chips.
